First Time Clients

Welcome to Crossroads Counseling. We are committed to assist you in the most professional, caring way possible.

Please read about our official greeters, if you are afraid of dogs, please let us know before your first visit. ( I added this because I was jst reading about them - your call I will take this out)

Please take a clipboard (one for each person) from the counter, have a seat and fill out the paperwork while you wait.

You will find a Client Information sheet, a Privacy Act form and the Contract. On the Contract, please initial the boxes throughout the contract. Give the completed paperwork, along with your insurance card, to your therapist.

It is likely your therapist is in session with another client. Once you have completed the paperwork, please sit back and enjoy the magazines, books or puzzles. We will be with you as soon as possible.

Feel free to ask any of us to assist you as needed. Restrooms are down the hall on the left.
